Q. What is the purpose of the survey?

A.   EPA and DEQ regulations require us to regulate certain businesses that discharge
substances into the sewer system that could damage the treatment system or pass through the system and into the Willamette River. This survey is part of our effort to identify those businesses.

Q. Why did I receive the survey?

A. Surveys are sent to all new businesses based on the City of Newberg business license database. Surveys are also sent to some classifications of existing businesses on a two year or five year rotation.

Q. We run a business out of our residence. Do we need to fill out the survey form?

A.   Yes, but not all of it. Fill out Section A, then check the “Yes” box for the question asking if the business is operated out of your residence. Skip to the bottom, sign the form, and return it to us.

Q. We don’t have an office or facility in Newberg. Do we still need to fill out the survey form?

A.  Yes, but, again, not all of it. Fill out Section A, then check the “No” box for the question in Section B asking if the building is connected to the Newberg sewer system. Skip to the bottom, sign the form, and return it to us.

Q. Why do we need to fill out the survey form? We don’t use any chemicals or dump anything harmful into the sewer system.

A.  That’s great! But we don’t know that until you tell us. Please fill out the survey form and return it to us.

Q. We just rent office space. Shouldn’t the owner of the building fill this out?

A. No, we’re surveying businesses, not locations. Please fill out the survey form to the best of your ability and return it to us.

Q. We run more than one business out of the same address. Do we need to fill out a survey form for each business?

A.  Yes. We need a signed form for each business for our records.

Q. We sent in the survey form, but got another one with a “second notice” letter. Why?

A.   We may not have received the first one, or your survey and the second notice letter may have crossed in the mail. Email or phone us to confirm that we received your survey. (Our contact information is on the survey form).

Q. Can we email or fax the survey to you?

A.  Yes. Either is acceptable as long as the survey form is completed and signed.

Q. The survey was addressed to our residence, but we don’t run a business. What should we do?

A.   It may be that a previous occupant of your home ran a business out of their residence. Or we may have made a mistake in addressing the survey. Contact us by phone or email and we will take care of this.

Q. The survey was sent to our address but was addressed to a different business. Can we ignore the survey?

A.   If you received only a survey addressed to the other business, fill out the survey with your business information and return it to us, even though it wasn’t addressed to you. Note on the form that the business the survey was addressed to is no longer at that address.

 If you received one survey addressed to your business and second one for the other business, fill out the first and write “closed” or “moved” on the second, and send both to us. 

Q. We don’t use any hazardous chemicals. Why did we get the RCRA Hazardous Waste Notification Requirements letter?

A.   We’re required to send the RCRA notification letter to all survey recipients.

Q. What is a NAICS or SIC code and how do I find it?

A. The NAICS and SIC codes categorize companies by their business activity. The primary code represents primary revenue for the company. SIC and NAICS codes create uniformity and comparability of various business activities.
