 Team Form: Please print off the Email conformation you receive and mail in your $300 Team Fee to Newberg Fire, 414 E 2nd St, Newberg, OR 97132. Make Checks payable to NVFD. 

 

IMPORTANT: By filling out this form you have NOT reserved your spot in the Toy and Joy Golf Tournament. Newberg Fire must receive your Team Fee to ensure your teams spot.
